Planning for your discharge is important – it helps to ensure that you have all the information you need to leave hospital safely and confidently.
Please ensure you have arranged for someone to take you home and care for you on discharge.
Discharge is at 10 am.
If you are unable to be collected by 10 am, you may be asked to move to the transit lounge.
You may discharge yourself from hospital or from treatment at any time. However, you do so at your own risk and the hospital will ask you to sign a self-discharge form.
Your discharge planning includes transport home, help at home, organising the right equipment, and managing your ongoing health care. Find out more about discharge planning before you leave.
